Take vitamin D with a meal. Take iron away from coffee. Take B12 on an empty stomach. Take this in the morning, that at night.
Supplement advice can make timing sound almost as important as the supplement itself.
But what does the evidence actually tell us?
The answer is more interesting than simply finding the perfect time of day.
A review of 84 studies found that routine vitamin and mineral supplementation offered generally healthy adults little or no protection against cardiovascular disease, cancer or premature death. That does not make supplements irrelevant – they have clear clinical uses, particularly where deficiency or increased nutritional requirements are established – but it does put the increasingly detailed advice around supplementation into perspective.
Because when supplementation is appropriate, timing can affect absorption. But absorption is not the same thing as clinical benefit.
Iron: a good example of the difference
Iron is particularly sensitive to what is consumed alongside it.
In a small study of 34 iron-depleted women, taking a 100mg iron supplement with coffee reduced absorption by 54% compared with water. Adding 80mg of vitamin C increased absorption by 30%.
On the surface, this seems to support the familiar advice to avoid coffee and take iron with vitamin C. But there is a catch.
A randomised trial involving 440 adults with iron deficiency anaemia found that iron supplementation alone was just as effective as iron combined with vitamin C at improving haemoglobin and iron stores.
So, although vitamin C changed how much iron was absorbed, it did not appear to change the clinical outcome. That distinction matters.
It is easy to assume that if a study shows a nutrient is absorbed more efficiently, the intervention must therefore be more effective. The evidence does not always support that conclusion.
Vitamin D tells a similar story
Vitamin D provides another example of absorption being influenced by circumstances rather than simply the time on the clock.
Because it is fat-soluble, vitamin D is absorbed more effectively when taken with dietary fat. One randomised study found greater absorption of vitamin D3 when it was taken with a meal containing fat than with a fat-free meal.
But the study does not tell us that breakfast is better than dinner, or that there is a particular hour at which vitamin D should be taken.
It tells us something more useful: the composition of the meal can matter more than the time of day.
That may sound obvious, but it cuts through much of the increasingly elaborate advice around supplement timing.
So, does timing matter?
Sometimes. But probably not in the way the supplement industry would have us believe.
For some nutrients, food, drinks and other substances can have a measurable effect on absorption. Iron is a particularly clear example, while vitamin D absorption is influenced by dietary fat.
Other supplements have much less evidence supporting a meaningful time-of-day effect.
And perhaps most importantly, a change in absorption does not automatically translate into a meaningful improvement in health.
That is why evidence around supplementation needs to be interpreted at several levels: absorption, bioavailability, biomarkers and, ultimately, clinical outcomes.
The question should not simply be whether a supplement is absorbed slightly better at one particular time. It should be whether that difference actually matters.
For healthcare and life sciences organisations, this is an important distinction as interest in personalised nutrition and preventative health continues to grow. More precise advice is not necessarily better advice if the underlying evidence does not demonstrate a meaningful clinical benefit.
So before worrying about whether a supplement belongs in the morning, evening, with food or without it, there is a more fundamental question: Is there good evidence that taking it will make a meaningful difference in the first place?
